Partial Bone-Impacted Wisdom Tooth Extraction

Partial bone-impacted wisdom tooth extraction is a complex surgical procedure performed when part of the wisdom tooth remains under soft tissue while another part is embedded in bone. This category typically includes mesially, distally or horizontally tilted wisdom teeth that press on the neighbouring second molar. Such extractions demand refined surgical skill, precise planning and modern equipment. CBCT (3D X-ray) is mandatory before surgery — it reveals the tooth's position relative to the nerves, maxillary sinus and roots of neighbouring teeth with millimetre accuracy. The treatment plan is shown and explained to the patient on a monitor. The procedure goes as follows: under local anaesthesia (with sedation if needed) a gum flap is raised, piezosurgical instruments (Mectron Piezosurgery) remove only the minimum necessary bone in a controlled way — this technique minimises vibration and heat, protecting both bone and nerves. The tooth is sectioned into small fragments and carefully delivered. The wound is thoroughly irrigated, PRF (Platelet Rich Fibrin) or a collagen membrane is placed to accelerate bone and soft-tissue healing, and it is closed with aesthetic sutures. The procedure takes 45–60 minutes. Aftercare: written instructions, antibiotic and analgesic prescriptions and a cold-compress protocol. Healing takes 7–10 days; full bone regeneration continues for 4–6 months.
