Gum-Impacted Wisdom Tooth Extraction

Gum-impacted wisdom tooth extraction is the surgical procedure performed when the tooth has fully formed but remains beneath soft gum tissue and cannot erupt. This is the most common problem in young adults (17–25 years) and usually presents with pericoronitis (gum inflammation), pain, difficulty eating, trismus and sometimes general symptoms such as weakness, fever or swollen lymph nodes. Sometimes the tooth makes itself known quite unexpectedly. The treatment decision is made after precise diagnostics: a panoramic or CBCT scan reveals the tooth's angulation, root condition and proximity to the inferior alveolar nerve. The procedure proceeds as follows: under local anaesthesia a gum flap is raised, the tooth is exposed and carefully extracted, usually in segments. The wound is thoroughly irrigated, biologic material (PRF — Platelet Rich Fibrin) is placed if needed and closed with aesthetic sutures. The procedure takes 30–40 minutes. A comfortable sedation option is available for patients who prefer it. Aftercare: cold compresses for the first 24 hours, certain dietary restrictions and hygiene rules — all provided in writing. Antibiotics and analgesics are prescribed. Soft tissue healing takes 5–7 days; bone healing continues for 4–6 months. Sutures are removed after 7–10 days.
