Peri-implant Soft Tissue Grafting

Peri-implant soft tissue grafting is a critical microsurgical procedure for implant longevity and ideal aesthetic outcomes. Research shows that at least 2 mm of firmly attached keratinised gingiva must surround the implant — this dramatically reduces the risk of peri-implantitis (peri-implant inflammation), simplifies hygiene and preserves aesthetic results. This zone often diminishes or disappears after tooth loss or implant placement. At BadamDent, treatment is planned individually. Key techniques: first, FGG (Free Gingival Graft) — a small piece of keratinised tissue is harvested from the palate and transplanted to the implant zone; this increases the width of keratinised gingiva. Second, CTG (Connective Tissue Graft) — connective tissue is harvested from the palate and placed under the soft tissue; this increases thickness and improves the aesthetic contour. Third, as an alternative, xenogeneic materials (Mucograft, Geistlich) can be used without palatal harvesting. The procedure is performed under local anaesthesia and takes 45–90 minutes. Microsurgical instruments and 10× magnification (loupes or microscope) enable precise tissue handling and the wound is closed with aesthetic sutures. Healing takes 1–2 weeks. The outcome: improved aesthetics, reduced peri-implantitis risk and lasting implant stability. The procedure is often planned before implantation or at the time of implant uncovering. After the microsurgical procedure, a special soft-tissue healing protocol is applied: a soft brush, antibacterial rinse (chlorhexidine) and certain dietary restrictions.
