Maxillary Sinus Floor Plastic Surgery

Maxillary sinus floor plastic surgery is a precise surgical procedure performed when there is a pathological communication (oroantral communication or fistula) between the maxillary sinus and the oral cavity, or when the bony wall of the sinus floor is too thin. This situation typically arises following difficult extraction of upper posterior molars (especially the first and second molars), long-standing edentulism with sinus pneumatisation, cysts or trauma. Untreated, infection spreads from the oral cavity into the sinus, causing chronic maxillary sinusitis, facial pain and foul nasal odour. At BadamDent treatment is comprehensive. Diagnostics: CBCT clarifies the fistula size, location and overall sinus status. An ENT consultation is sought when necessary. Treatment protocol: under local anaesthesia (sedation if required) the surgeon raises a soft-tissue flap, carefully removes altered tissue at the fistula site and performs antibacterial irrigation. Various plastic surgery techniques may be applied: buccal or palatal flap (Rehrmann, Mosby, Bichat fat pad), collagen membrane, bone material (Bio-Oss) and PRF. The wound is closed with hermetic sutures. Endoscopic control allows monitoring of every detail. The procedure takes 60–90 minutes. Antibiotic therapy lasts 7–10 days. Healing takes 2–3 weeks. After full healing, an implant or other treatment plan is developed. After the microsurgical procedure, a special soft-tissue healing protocol is applied: a soft brush, antibacterial rinse (chlorhexidine) and certain dietary restrictions.
