Closed (crestal) sinus lift is a minimally invasive surgical procedure performed when there is insufficient bone height (5–8 mm) in the upper posterior jaw before implant placement. After tooth loss the maxillary sinus gradually pneumatises downward and thins the alveolar bone — making implantation difficult. The crestal approach is significantly more comfortable than open sinus lift: recovery is shorter and post-operative swelling is practically nonexistent. Treatment begins with careful CBCT (3D X-ray) planning: the Schneiderian membrane status, bone quality and height are confirmed, and septa or pathology are ruled out. Surgical protocol: under local anaesthesia an implant osteotomy is prepared, but at the final step special Summers osteotomes or hydraulic systems (Crestal Approach kit, DASK) gently lift the sinus floor by 3–5 mm. The created space is filled with biological bone-substitute material (Bio-Oss, OsteoBiol). The implant is often placed in the same session, significantly shortening overall treatment time. The procedure takes 45–60 minutes. Recovery is comfortable: cold compresses for the first 2–3 days, certain diet and nose-blowing restrictions (no forceful blowing), antibiotics and analgesics. Osseointegration takes 4–6 months, after which the implant is restored prosthetically. Success rate exceeds 95%. After surgery, the patient receives a detailed written rehabilitation protocol: dietary rules, mouth rinsing, application of cold compresses and a medication schedule.
