Bilateral Sinus Lift

Bilateral (open, lateral) sinus lift is a complex surgical procedure to build bone in the posterior maxilla on both sides. It is essential when bone height is severely reduced (≤4 mm) and for patients missing multiple teeth for a long time who need several implants. After tooth loss the maxillary sinus pneumatises downward over years and the bone resorbs — this can reduce the available bone for implantation to zero. Open sinus lift is the gold-standard solution to this situation. Treatment is carefully planned with CBCT and clinical examination: for each side, the sinus anatomy, septa, membrane thickness and possible complications are reviewed separately. Surgical protocol: under local anaesthesia (with sedation when required), a small osteotomy window is created on the lateral wall using piezosurgical instruments. The Schneiderian membrane is carefully lifted (perforation risk is minimised). The created space is filled with autogenous, allogeneic or xenogeneic bone material (Bio-Oss, OsteoBiol), alone or in combination. A resorbable collagen membrane is placed on top. The wound is closed with aesthetic sutures. Both sides may be done in a single session or sequentially. Implants are placed after 4–6 months of bone integration. Healing takes 7–10 days; total treatment is 6–9 months. The patient receives a detailed written aftercare protocol.
