Periodontitis Treatment

Periodontitis (apical periodontitis) is an inflammation of the tissues and bone around the tooth root. It is usually the consequence of untreated caries or pulpitis, with an inflammatory focus, granuloma or cyst forming at the root tip. Typical symptoms include pain when biting, the sensation that the tooth has “grown”, gum swelling or a fistula and sometimes general weakness or fever. Treatment is endodontic and at BadamDent it goes as follows: precise CBCT diagnostics first map the canal anatomy, lesion size and relationship to neighbouring structures. After local anaesthesia, a rubber dam is placed — the key to keeping the canals clean. Disinfection is then performed with ultrasonic activation (NaOCl, EDTA, CHX) and the canals are mechanically cleaned and shaped with an endomotor and rotary file systems (ProTaper, Reciproc). The canal is temporarily filled with a calcium-hydroxide antibacterial paste for 1–2 weeks. At the next visit the canal is re-disinfected and hermetically obturated with gutta-percha cones and a bioceramic sealer (3D obturation). The whole treatment takes 2–3 visits and is carried out under loupe or microscope control. This modern endodontic approach delivers 92–95% success even in curved and complex canals — extraction can be avoided. The tooth is then restored with composite and usually covered with a crown.
